Shiosaki Physical Therapy currently may provide services for the following insurances:

• Coordinated Care: WA Apple Health Plan and Ambetter Plan
• Department of Labor and Industries, WA
• First Choice Health plans
• Humana
• Kaiser Permanente Plans including Kaiser Permanente Medicare Advantage and HMO plans
• Medicare B
• Molina
• OptumHealth Care Solutions Plans: United Healthcare plans – Commercial, Employer based, AARP Medicare Advantage from United HealthCare; Nexus and Zelis plans
• Premera - (vestibular/vertigo patients only)
• Regence WA: includes Federal Blue Cross Blue Shield plans
• Tricare: Shiosaki Physical Therapy is credentialed with Tricare, however Tricare limits the number of clinic contracts in the region so we may only see Tricare patients at the “out of network reimbursement rates”. You will pay a larger percentage of the bill here than at an “in network contracted clinic”.

Please call your insurance to see if Shiosaki Physical Therapy is listed as a provider. If not, ask if the insurance has out of network benefits. We may see private pay – cash patients, though I would like to attempt to provide services through your insurance to help you avoid financial stress.
